THE GOODS AND SERVICES SHOWN BELOW ARE THOSE WE CAN PROVIDE TO OUR CUSTOMERS. YOU MAY CHOOSE ONLY THE ITEMS YOU DESIRE. HOWEVER, ANY FUNERAL ARRANGEMENT YOU SELECT WILL INCLUDE A CHARGE FOR OUR BASIC SERVICES AND OVERHEAD. IF LEGAL OR OTHER REQUIREMENTS MEAN YOU MUST BUY ANY ITEM YOU DID NOT SPECIFICALLY ASK FOR„ WE WILL EXPLAIN THE REASON IN WRITING ON THE STATEMENT WE PROVIDE DESCRIBING THE FUNERAL GOODS AND SERVICES YOU SELECTED.

Our basic service charge includes, but is not limited to, staff to respond to initial request for service, conducting arrangement conference with family or responsible party, clergy, cemetery, or crematory and others as required. Placement of obituary or death notices, recording vital statistics and filing of necessary certificates, permits, and assistance with forms and notary service. Retention and care of remains prior to commencement of post death activities. This fee for basic services will be added to the total costs of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains).

Embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as a direct cremation or immediate burial.

Procurement of death certificate from location other than place of death or medical examiner's cremation authorizations, or any other courier service locally.

(Memorial items not in stock may incur an express shipping charge)

This charge includes basic services of funeral director and staff, transfer of remains to the funeral home, embalming, minimum shipping container and local ground transportation to airport. This charge does not include any services or staff for visitation or funeral ceremony prior to forwarding the body, or shipping charges. (You may add dressing, cosmetic application, casketing and the casket of your choice).

This charge includes basic services of funeral director and staff, transportation from airport, shelter of remains, and local transportation (hearse) to cemetery or crematory. This charge does not include visitation or any ceremony.

Our charge for immediate burial includes basic services of funeral director and staff, transfer of the remains to the funeral home, and local ground transportation to the cemetery. It does not include embalming or use of the facilities for viewing or funeral ceremony or any merchandise. Refrigeration or burial pouch additional if necessary.

Our price for direct cremation includes basic services of funeral director and staff, transfer of the remains to the funeral home, personal identification of deceased, crematory fee, and transportation to the crematory. It does not include embalming, or facilities for viewing or funeral ceremony, medical examiner's fee and permit, or any merchandise. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). An alternative container is required for direct cremation.

Payment in full is due at the time of contract signing or before the local burial or cremation service.

• We accept the following forms of payment for local and domestic services.
• Credit/Debit card transactions by VISA, MasterCard, Discover or American Express.
• Cash, money order ,cashier's check or certified bank check.
• Personal check by contract signer with proper identification and social security number.
• Insurance assignments at our sole discretion may be honored in lieu of full payment at contract signing.
• Insurance assignment processing fee of $150.00 will apply.

We accept the following forms of payment:

Estates:

We do not invoice estates for payment, unless the attorney of record for the estate will sign the funeral contract. Interest paid on the unpaid balance after 30 days may apply. We encourage the responsible party or personal representative to apply for reimbursement from the estate.
